Leann Cheney (name changed for confidentially) was injured in the course of her employment in Lewisberry, PA. Indemnity Insurance Company of North America, the workers’ compensation insurance carrier issued a Medical-only Notice of Temporary Compensation Payable (NTCP) on November 12, 2014 for Ms. Cheney’s September 14, 2014 injury described as a right shoulder strain. A Notice of Compensation Payable was then issued on January 14, 2015 which described the injury as a right shoulder strain / sprain.

Claimant underwent treatment for the injury including surgery on July 8, 2008 in the nature of a fusion at the L4-5 vertebrae. The Notice of Temporary Compensation Payable had merely described the work injury as a strain.
Employee LD, a resident of Lebanon, Lebanon County, suffered a work injury on June 6, 200 while employed by Mack Employment Services. Claimant was injured when she fell on her hands and knees, rupturing a disc in her back. Claimant has treated with Dr. Stuart Hartman, MD, Dr. Dinello, DMD and Dr. Rick Yazinski, DMD.
Employee CD, a resident of Highspire, Dauphin County, suffered a work injury on October 14, 2006, while employed by VFW Post 1718. The workers’ compensation insurance company denied payment of massage therapy for the injured worker.
